You may need to complete a Voluntary Disclosure Form if you have any prior convictions, cautions, diversions, good behavior bonds or excessive driving offences. This form can be completed to determine if the prior offences will be detrimental to your application to join Victoria Police. Please do not email or fax the form back. The form needs to be sent back to Victoria Police Recruitment Branch, 637 Flinders Street, Docklands, VIC, 3008.
